I saw it right before a Dr. appt that I had scheduled with my son who's 15 and wants to get vaccinated so that he qualifies to become a volunteer EMT. I printed out this form and brought it to the appt for the Dr. to go over with my son. It's at least given him pause, for now. Keep up your important work! <3
Requiring parental consent is an absolute bare minimum. I'm tempted to go even further and prohibit ANY consent by a parent/guardian, an official or the child him/herself. The drawback is that it would prohibit all medical care for children, and I'm not sure how to get around that. One might think that limiting that requirement to EUAs would be sufficient, but it's clear now that even fully approved drugs and biologics can not be trusted after the Comirnaty approval. At least by requiring parental consent we increase the chances consent will be given by someone who truly cares for the child, though unfortunately many parents will obey self-interested authorities. It is a dilemma, but at least if parents read this form they'll be more likely to behave rationally.
A reminder that the onus is on the govts to prove that the vaccine is safe, effective and necessary. No govts, or advocates, can meet those 3 essential requirements.

Thank you for making this very comprehensive document available. However I think there is still one more point that could be added.
