Jessica, thank you so much for posting this when you did. I saw it right before a Dr. appt that I had scheduled with my son who's 15 and wants to get vaccinated so that he qualifies to become a volunteer EMT. I printed out this form and brought it to the appt for the Dr. to go over with my son. It's at least given him pause, for now. Keep up your important work! <3
Thank you for making this very comprehensive document available. However I think there is still one more point that could be added.
