My mom who died in 2020 at age 98 was in a "memory unit" for the final 4 years of her life. She was surrounded by people who were shells. What ever cognitive faculties she had were quickly extinguished as she sat there and vegetated. What she needed was stimulation and people to talk to so that she could hold onto those fading memories. She would sleep a lot and I would ask her what she was dreaming about and she would say, "when I was young". It breaks my heart knowing that what we thought was elder care was as far from care as one could get.

Most in MC die within 6 months and my guess is the depression kills them if the plaques don't. At my dad's facility, staff "put them to bed" by 6. They became frustrated with me because I was taking him out and staying until 730 or 8. I regret not forcing him to move in with me, gave in to sibling pressure re "he'll be happier around a lot of people", only to discover that the people were all on the road to death via depression and lack of stimulation. Nurses control this environment, not activity therapies, and they are generally not looking for ways to enhance cognition and maintain "compliance", often via medication. "Residents" don't go outside (to a patio space), they constantly look at the same walls and nurses are more concerned with cya, "fall risk", than with maintaining any physical strength. Even in a $10k/month facility, the food is bland, overcooked and small portions (so there's no weight gain as you sit and sit and sit). If any of us were in MC for six months, we'd want to be dead if it didn't kill us. If there's any way to avoid this, keep your elders out.
I just read a report that says, in Canada, basically all care facilities like Hospices must, and I repeat, MUST, offer their clients MAID, or suffer the consequences. As in, no funding, and if that is not needed, then they will shut it down and take it over.

Educating the current professors, doctors, and other multi-letter holders of “respect me” titles would also be nice. Most of them are at the age when their brain resists new information. Introducing a mandatory examination in their field of specialisation every 2 years would be great. In this way, we could repopulate the damaged medical circles at a fraction of cost and with measurable benefits in sight.
